How the service works
The system supports existing MDT pathways rather than requiring teams to adopt a new referral portal or significantly change established working practices.

Complete the required information
The form guides the referrer through the clinical and administrative information requested by that MDT.
Send it into the existing workflow
The referral is converted into a clear, consistent format and emailed to the designated MDT team. Patient information is not retained by the website.

Designed with clinical governance in mind
The platform has been developed around a data-minimising architecture. It provides a structured route for sending referral information into an established clinical mailbox without creating a separate store of patient records.

Implementation is currently offered without charge as part of the NHS Yorkshire and Humber Regional Impact Project Fellowship work.
